WHAT YOU NEED TO KNOW

1. Parents do not need to be present at the clinic if you complete ALL consent forms.
2. Students ages 0-18 are eligible if they are uninsured, have Medicaid or are Native American/Alaska Native.
3. At this time, we are not able to provide the COVID-19 vaccine, but we can help you find out where to get it for your teen.
